   Your initial visit to Pretty River Foot Clinic is important to us, and will help establish a solid foundation for our relationship with you. During your first visit, a complete medical history and thorough assessment will be performed to ensure that proper treatment plan is created. Being well prepared for your appointment will ensure that our team has all the required information to provide the best possible care for you. 

New Patient Reminders:

• Please ensure new patient form(s) are completed;
• Please check with your health plan-benefits for coverage information, and bring this information with you;
• Please bring an updated medication list (prescription and over the counter meds);
• Please bring shoes worn most often: work, exercise, indoors, etc...
• Please remove any nail polish from your toenails;
• Please bring any pertinent reports from your doctor, and/or imaging studies (x-ray, MRI, CT scan) if available;
• Please call at least 24 hours in advance if you are unable to keep your appointment.
